ABSTRACT

In recent years, the phenomenon of glass transition has been gradually applied to the field of pharmaceutics. And it exhibits important influences on multiple operating units of pharmaceutical preparations, and the properties and storage of pharmaceutical intermediates and products. At present, it has been widely used in the process of preparations such as drying, granulation, coating, tableting, holt-melt extrusion, cryogenic comminution, and so on. Meanwhile, it showed guiding significance for the process of preparation intermediates and their products, such as solid dispersion, microcapsule, liposome, particle, tablet, and other preparation intermediates and their products. Therefore, this article conducts a detailed analysis and systematic summary of the application guidance of the phenomenon of glass transition in the preparation process, and its influence on the preparation intermediates and products, so as to provide theoretical guidance for preparation production and product storage.

ABSTRACT

Objective::Near infrared spectroscopy was used to detect the concentration density (25 ℃), solid-containing content, rhein content and glycyrrhizic acid content of compound Dahuang decoction. Method::The concentrated liquid of compound Dahuang decoction was determined by near infrared optical fiber transmission spectrometry. The contents of rhein and glycyrrhizic acid were determined by HPLC. Fifty-one samples were used for internal cross-validation, and partial least square regression was used to establish correction models between near-infrared spectrum and density, solid-containing content, rhein content and glycyrrhizic acid content, respectively. Ten unknown concentrated liquid samples were collected for external validation and prediction. Result::The external validation complex correlation coefficients between near-infrared spectra and density, solid-containing content, rhein content and glycyrrhizic acid content of the concentrated liquid of compound Dahuang decoction were 0.995 9, 0.999 6, 0.997 0 and 0.992 2, and the root mean square error of prediction (RMSEP) values were 2.50×10-3, 0.17, 7.57 and 67.10, respectively. Conclusion::The near infrared spectroscopy is suitable for the determination of evaluation indexes of the concentrated liquid index of compound Dahuang decoction, and has the characteristics of rapid, simple, stable and reliable.

ABSTRACT

OBJECTIVE: To review systematically the effect and safety of tofacitinib for patients with moderate to severe psoriasis. METHODS: Relevant studies were published until May 16, 2018 were identified through The Cochrane Library, PubMed, Embase, Clinical Trials.gov, CNKI, Wanfang, CBM and other web knowledge databases. Two reviewers independently screened literatures according to the inclusion and exclusion criteria, extracted data, and assessed the methodological quality of the included studies. Then we performed statistical analyses using the Review Manager 5.2 and Stata 12.0 software. RESULTS: Eight randomized controlled trials that compared tofacitinib vs placebo were included in the study. The final Meta-analysis included a total of 3 308 patients with psoriasis. Tofacitinib was associated with reductions in psoriasis area and severity index 75% (RR 3.27, 95%CI1.79-5.98, P=0.000 1), psoriasis area and severity index 90% (RR 12.61, 95%CI7.66-20.76, P<0.000 01), physician′s global assessment (RR 4.38, 95%CI3.51-5.47, P<0.000 01). A safety analysis showed that tofacitinib increased the risk of hypercholesterolemia (RR 2.57, 95%CI1.22-5.38, P=0.01) and tended to associate with increasing the risk of herpes zoster (RR 3.20, 95%CI0.86-11.91, P=0.08). CONCLUSION: Tofacitinib is effective and relatively safe for patients with psoriasis. Therefore, tofacitinib may be a promising treatment option for patients with moderate to severe psoriasis who had previously an inadequate response to the conventional synthetic treatment.

ABSTRACT

<p><b>OBJECTIVE</b>To observe the improvements of synchronous treatment of bilateral scalp acupuncture and rehabilitation training on activities of daily life in patients with cerebral infarction at acute phase, so as to compare the efficacy differences between scalp acupuncture at bilateral and affected side as well as differences between synchronous and non-synchronous treatment.</p><p><b>METHODS</b>Ninety patients of acute-phase cerebral infarction with motor dysfunction were randomly divided into three groups. The observation group was treated with synchronous treatment of scalp acupuncture at the Dingzhongxian (middle line of vertex), bilateral Dingnieqianxiexian (anterior oblique line of vertex-temporal) and bilateral Dingniehouxiexian (posterior oblique line of vertex-temporal) and rehabilitation training; the control group A was treated with synchronous treatment of affected scalp acupuncture at the Dingzhongxian, affected Dingnieqianxiexian and affected Dingniehouxiexian and rehabilitation training; the control group B was treated with bilateral scalp acupuncture for 4 h, followed by rehabilitation training. All the patients took the treatment once a day, and 6 days for a course of treatment for total of 4 courses. The modified Barthel index (MBI), activities of daily living (ADL) and Fugl-Meyer motor assessment (FMA) were used to perform efficacy assessment before treatment, in the 14th days of treatment and in the 28th days of treatment in three groups.</p><p><b>RESULTS</b>After treatment, three indices at each time point were superior to those before the treatment in three groups (all P<0.01) ; the improvements of ADL and FMA in the observation group after 28 days of treatment were superior to those in the control group A and control group B (all P<0.05), and the improvement of MBI was superior to that in the control group B (P<0.05).</p><p><b>CONCLUSION</b>The synchronous treatment of bilateral scalp acupuncture and rehabilitation training could significantly improve the activities of daily life and motor function in patients with cerebral infarction at acute phase, which is superior to scalp acupuncture at affected side and non-synchronous treatment.</p>

ABSTRACT

Objective To study the accuracy and the clinical significance of sentinel lymph node biopsy (SLNB)after neoadjuvant chemotherapy for breast cancer.Methods A total of 90 patients with StageⅡorⅢbreast cancer and negative axillary node after neoadjuvant chemotherapy were enrolled in the study.Mapping proce- dure and SLNB were performed using methylene blue injected at the site of the primary breast cancer,followed by the axillary lymph node dissection.Results The sentinel lymph node(SLN)was successfully identified in 82 out of 90 patients(91.1%).The number of sentinel harvested nodes ranged from 1 to 4(average 1.6).The accuracy of SLNB to predict the axillary lymph node status was 93.9 %(77/82),the sensitivity,positive predictive value,nega- tive predictive value and false negative rate were 87.5 %(40/45),100 %,88.1% and 11.1%(5/45),respectively. The SLN identification rate tended to be higher and false negative rate tended to be lower in patients with T2 prima- ry tumor before neoadjuvant chemotherapy.Conclusion Our study indicated that SLNB after neoadjuvant chemotherapy in patients with StageⅡorⅢbreast cancer had a similar effect as SLNB in non-neoadjuvant studies. SLNB was considered to be able to accurately predict the axillary lymph node status in patients with T2 primary tu- mor before neoadjuvant chemotherapy.

ABSTRACT

<p><b>OBJECTIVE</b>To search for an effective method for treatment of chronic hepatitis B.</p><p><b>METHODS</b>One hundred and twenty-three cases were randomly divided into a treatment group (n = 63) and a control group (n = 60). The treatment group were treated with injection of Huangqi injectio and Danshen Injectio into Ganshu (BL 18) and Zusanli (ST 36), once every other day; and the control group were treated with oral administration of Gankangning tablet and fufang yiganling tablet. The clinical symptoms, hepatic function, hepatitis B e antigen (HBeAg), hepatitis B virus-desoxyribose nucleic acid (HBV-DNA) were compared between the two groups.</p><p><b>RESULTS</b>The total effective rate was 93.7% in the treatment group and 76.7% in the control group with significant difference between the two groups (P < 0.01); and the treatment group in hepatic function and the effects of turning negative for HBeAg and HBV-DNA was better than the control group (P < 0.01).</p><p><b>CONCLUSION</b>Acupoint-injection has a better therapeutic effect on chronic hepatitis B.</p>
